Three foundational concepts shape classical homeopathic practice. The aggravation describes the temporary worsening of symptoms that often signals a remedy is acting. The proving is the systematic testing of a substance on healthy individuals that builds the materia medica. And miasmatic theory — Hahnemann's most ambitious contribution — describes the underlying disease tendencies that predispose us to chronic illness. Together they form the deeper grammar of homeopathic prescribing.
Understanding Aggravations
An aggravation is a temporary worsening of symptoms after taking a remedy. Far from a side effect, a well-judged aggravation often signals that the remedy is acting on the vital force and that the organism is responding. Hahnemann distinguished several forms, each with its own clinical meaning.
Initial aggravation
A brief intensification of the patient's existing symptoms in the hours or days after the dose. It indicates the remedy matches the case closely and the vital force is stirred. It typically subsides on its own and is followed by improvement.
Therapeutic aggravation
A temporary return of old, long-suppressed symptoms — sometimes from years past. This is generally a favourable sign that the remedy is reaching deeper layers and the organism is re-ordering its symptom hierarchy in reverse chronological order.
Understanding Provings
A proving (from the German Prüfung, "test") is the systematic administration of a substance to healthy volunteers to discover the symptom picture it can produce — and therefore, by the law of similars, the symptoms it can cure. Provings are the empirical foundation of the homeopathic materia medica.
Hahnemann's methodology
Hahnemann insisted provings be conducted on healthy individuals, using repeated doses of the substance until symptoms appeared, with every sensation meticulously recorded. He published his first proving of Cinchona in 1796 and went on to prove nearly 100 remedies himself.
Healthy provers, not patients
By testing on the healthy, provings isolate the remedy's pure action from the disease itself. Symptoms produced in a prover become the reliable guide to what the same remedy can cure in the sick — the practical expression of similia similibus curentur.
The basis of the materia medica
Every entry in the materia medica — mental, emotional, and physical symptoms across the body's regions — traces back to a proving. Modern double-blind proving protocols continue to extend the literature, refining the symptom pictures Hahnemann began.
Miasmatic Theory
In his later years Hahnemann observed that many chronic diseases resisted well-chosen remedies, returning in altered form. He attributed this to underlying disease tendencies — miasms — that predispose the organism to chronic illness. A miasm is not a specific disease but an inherited or acquired susceptibility that shapes how the vital force expresses disturbance.

Sycosis is associated with overgrowth and excess — warts, fig-warts, condylomata, and a history of suppressed gonorrhea. The sycotic constitution tends toward retention, congestion, and proliferative tissue changes, with symptoms that worsen in damp and improve in dry, open air.

Aggravations show as increased discharges, swelling, or proliferation of growths. Symptoms worsen in damp, cold weather and at the seashore, and improve with dry warmth and continued gentle motion.

The syphilitic miasm is marked by destruction — ulceration, necrosis, and deep tissue damage that progresses silently. The syphilitic constitution tends toward fixed, stubborn pathology, night pains, and a drive toward decay that does not resolve without deep-acting remedies.

The cancer miasm describes a constitution prone to tumors and growths, often with a family history of cancer or a pattern of suppressed grief and over-responsibility. The cancer constitution is conscientious, empathic, and tends to push through until pathology manifests as hard, indurated growths.
